IMPORTANT NOTICE FOR TENET JOB CANDIDATES WHO RESIDE IN CALIFORNIA

California Consumer Privacy Act (CCPA) and Personal Information Collected

 

In June 2018, Assembly Bill 375 was introduced as the California Consumer Privacy Act 2018. As part of this Act businesses are required to inform job candidates of the personal information that they collect or share with third parties, and the business purpose for which the information is used.

The purpose of this Job Candidate Privacy Notice (“Notice”) is to inform job candidates of Tenet Healthcare and its affiliated companies, including Tenet Physician Resources, MedPost/Care Spot and United Surgical Partners International (collectively, the “Company” or “we” or “us”) of the categories of Personal Information (as defined below) that we collect and the purposes for which we use such Personal Information.  

1.  Collection and Use of Candidate Personal Information

We collect, use, and disclose Personal Information about job candidates for business purposes only and consistent with applicable laws.  Where we disclose Personal Information to third parties, we do so for the same business purposes described below and, where appropriate, we require that such parties maintain the confidentiality of the Personal Information and maintain appropriate systems and processes to ensure its security and protection.

For purposes of this Notice, “Personal Information” is information that relates to an identified or identifiable person. Personal Information does not include deidentified, aggregated, or combined information, which includes data that is not reasonably capable of identifying you or being linked to you. 

We may collect and use the following categories of Personal Information about you to when you inquire about and/or apply for employment with us:

Contact information: such as your name, home and work address, personal and work telephone numbers, personal and work email addresses.

 

We will use this information to open and maintain candidate records and communicate with you for regarding your interest in a job opportunity with us, your application for employment, or updates on your candidacy.

 

Job-related information and qualifications: such as position applied for, previous job roles, education, awards, qualifications, certificates, licenses, and any other information you choose to provide in your resumes and/or applications.

We will use this information to assess your suitability for a position with us and conflict of interest reporting, except to the extent prohibited by law.

National identification information: such as information contained in documents necessary to complete a Form I-9 (e.g., passport information, permanent resident card information, drivers’ license or ID card, social security account number, certification of report of birth, etc.), citizenship, any visa or other right to work information, national origin.

We will use this information to determine your eligibility to work and fulfill our obligations to relevant government authorities and, where permitted or required by law, to be compliant with, among other things, the Export Administration Regulations or International Traffic in Arms Regulations.

Travel-related records: loyalty reward programs, itineraries, flight, train, rental car, car service, hotel or other similar information.

We will use this information to arrange or reimburse travel where you require travel to interview with us or in connection with your candidacy for employment, contact you during travel, as necessary with travel service providers, or in an emergency situation.

Results of background checks and screening: such as education verification and criminal records and driving license checks.

We will use this to determine your suitability for certain job roles (to the extent permitted or required by law).

References: such as opinions and comments of any previous employers.

We will use this to determine your suitability for certain job roles.

Health and safety records: information relating to health and safety in the work place, accidents and near misses.

We will use this to address our legal obligations to you in relation to health and safety in the work place.

Information we obtain from monitoring: such as records of your use of our computer systems or the Internet, recordings from surveillance cameras on our business premises, and recordings of customer service calls.

 

We will use this to monitor your compliance with our internal policies and procedures, to investigate security breaches and misuse of computer equipment and systems, to protect the safety of contractors and third parties, and to protect our property from theft, vandalism and damage.

Audio/photo/video information: audio recordings, video recordings, or photos taken during the normal course of business or during voluntary business events

We may use this for any business-related purpose including, but not limited to, social media, advertising, training, internal or external marketing, and record-keeping.

Diversity information: such as your ethnicity/race, disability status, veteran status, gender, and sexual orientation.

We will use this to conduct equal opportunity and diversity analysis where permitted or required by law.

 

2.  Additional Purposes for Using Personal Information

In addition to the uses set forth above, we may also use and share the categories of Personal Information identified above:

  • To comply with applicable legal and regulatory requests and obligations (including investigations).
  • To establish or defend legal claims and allegations.
  • For security or the prevention, detection, or investigation of fraud, suspected or actual illegal activity, or other misconduct.
  • To seek advice from lawyers, auditors and other professional advisers.

3.  No Sale of Personal Information

     We do not sell personal information of our job candidates.
